A You may obtain a benefits enrollment form from an enrollment counselor/coordinator or your benefits office. If your employer utilizes online enrollment, you may be able to download a benefits enrollment form from your company Web site.

Q I've turned in my enrollment form, however, I forgot to add my dependent's information on the form. How can I add this?
A Find out how to contact FBMC by visiting the Contact Customer Care section of this website and request to be transferred to our Dependent Information Hotline (ext. 544). You must leave your information (name, employer and Social Security Number) along with your dependent information (name, date of birth, SSN and benefits for which the dependent is to be assigned).

Q Can I verify what benefits I currently have?
A Yes, you can confirm your benefits by first logging in to www.myFBMC.com using your User ID and Pin Number. Then you can click on the Benefits tab, and select Benefit Status from the drop down menu. Or, find out how to contact FBMC by visiting the Contact Customer Care section of this website.

Q When are my benefits effective?
A If you are re-enrolling during your benefits open enrollment period, your benefits effective dates are located on your enrollment form or within your benefits booklet. If you are a new hire, please check with your benefits office for your plan effective date.
