Find out how to contact FBMC by visiting the Contact Customer Care section of this website and request to be transferred to our Dependent Information Hotline (ext. 544). You must leave your information (name, employer and Social Security Number) along with your dependent information (name, date of birth, SSN and benefits for which the dependent is to be assigned). |

If you are re-enrolling during your benefits open enrollment period, your benefits effective dates are located on your enrollment form or within your benefits booklet. If you are a new hire, please check with your benefits office for your plan effective date. |
